I understand that these reports may include information regarding my criminal history, employment history, education verification, motor vehicle records, and other relevant background information.

I authorize courts, law enforcement agencies, former employers, educational institutions, and other sources to release information to Altaris Global.

I understand that I have the right to request a copy of my report and dispute inaccurate information.
